I would like a standard form to submit to the email address and the rest go to paypal. Is this possible? Some thing like this.
Name
Address
City State Zip
Phone
( a box to check) "Prayer"
"&" (centered)
(a box to check) "A one time donation of $(a box to fill in) (Ex. $5.00)"
"or" (centered)
(a box to check) "A monthly gift of $(a box to fill in) for (a box to fill in) months."
If you can help I appreciate it. I need the Name etc for send receipt to customer for tax deduction because this is a 503b Organization.
