Hi,
Is there any way to use the paypal calculator (as a base amount, say $5), and use the per item variable as an addition to it (and which variable do I use: "shipping", "shipping2", or "shipping_extra")?
Or will this work at all?

You need to use one or the other. "shipping" is the base amount for a specific product in your cart. "shipping2" is the amount for each additional quantity of that item. What if some items use the shipping calculator (flat rate, say) and a separate item uses the per item feature?
My specific problem is that most items can have their shipping rates calculated with the flat rate calculation, but one item is much to heavy to allow that. When the heavy and normal items are combined in a shopping cart, the heavy item's shipping rate overrides (set by the the shipping, shipping2, and shipping_extra hidden fields) the flat rate calculation.
The documentation implies that both the normal flat rate calc and the per item calc should apply.
